As a Registered Nurse, you will provide essential and directed nursing care to patients in their homes, as prescribed by physicians while adhering to all applicable laws and agency policies. You will coordinate comprehensive care plans with healthcare professionals to maintain continuity of care, facilitating communication among patients, families, and community resources.

Key Responsibilities:
• Deliver high-quality clinical services in accordance with infection control standards and your professional scope of practice.
• Collaborate with the patient’s care team from admission through discharge to ensure comprehensive care coordination.
• Conduct clinical nursing assessments in line with federal and state requirements as well as payer necessities.
• Verify patient eligibility and medical necessity for services according to payer and agency policies.
• Develop and regularly update individualized care plans in collaboration with community providers.
• Adjust the frequency and duration of care plans based on patient needs, with physician approval.

Required Qualifications:
• Current and unrestricted RN license in your state of practice.
• Valid driver’s license, vehicle insurance, and access to a reliable vehicle or public transport.
• Current CPR certification.
• Ability to adapt to diverse home situations, respecting age, race, creed, color, sex, disability, or financial status of the client.

Preferred Qualifications:
• At least 1 year of RN experience.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

We offer a competitive salary ranging from $28.27 to $50.48 per hour based on full-time employment, along with a comprehensive benefits package, incentive programs, and retirement plan options. All benefits are subject to eligibility requirements.
