Overview Compensation range - $85,000 - $105,000 annually. Your main duties will include conducting patient visits and acting as the primary liaison among various care disciplines. You will oversee patient visit frequencies and engage with physicians for clear communication regarding patient conditions. Key responsibilities include: • Conducting accurate OASIS assessments • Managing current medication profiles • Timely reporting of lab values to physicians • Participating in weekly case conferences and monthly case manager meetings Qualifications To be eligible for this position, you must meet these qualifications: • Current Registered Nurse (RN) licensure in the state of employment • A minimum of one year of clinical experience is preferred • Proven knowledge and skills in contemporary nursing practices • A valid state driver's license and automobile liability insurance Additional Information Enhabit Home Health & Hospice is proud to be an equal opportunity employer, fostering an inclusive environment that values diversity and collaboration.
