In the role of Registered Nurse, you will provide essential nursing care to patients in their homes, as per physician directives and in accordance with applicable laws, regulations, and agency policies. You will play a vital role in coordinating comprehensive care alongside other health care professionals, ensuring continuity throughout the patient's care journey by communicating effectively with physicians, agency staff, patients, families, and community resources.

Key Responsibilities:
• Deliver top-tier clinical services within your scope of practice, adhering to infection control standards.
• Collaborate with the patient's care team to coordinate care from admission to discharge.
• Conduct clinical nursing assessments in alignment with federal/state program requirements and payer specifications.
• Verify patient eligibility and medical necessity for services as per payer and agency policies.
• Create and adjust individualized care plans with partner community providers.
• Ensure the frequency and duration of care plans effectively meet patient needs, initiating revisions with physician approval.

Required Qualifications:
• Valid and unrestricted RN licensure in the practicing state.
• Current driver's license, vehicle insurance, and reliable access to transportation.
• Active CPR certification.
• Ability to work effectively in diverse home situations, regardless of age, race, creed, color, sex, disability, or client financial status.

Preferred Qualifications:
• Over 1 year of RN experience.
• Capable of independent work.
• Strong communication, writing, and organizational abilities.
