Key Responsibilities: As a Registered Nurse in Home Health Care, your duties will include: • Conducting patient visits and coordinating care among various disciplines. • Acting as the primary point of contact for all aspects of patient care coordination. • Consulting with physicians and providing accurate reports on patient care. • Collecting OASIS data and ensuring medication profiles are current and accurate. • Reporting lab values to physicians in a timely manner. • Participating in weekly case conferences and monthly case manager meetings. Qualifications: To be considered for this exciting opportunity, you must: • Possess a current Registered Nurse (RN) license in your state. • Have at least one year of clinical experience (preferred). • Exhibit knowledge and skills in current nursing practices. • Hold a valid state driver's license and have automobile liability insurance.
