Home Health RN Case Manager Responsibilities
• Receive a $10,000.00 Sign-On Bonus (disbursed over the first year).
• Conduct admissions, case management, and follow-up skilled nursing visits for home health clients.
• Provide ongoing care and case management, including necessary follow-up as directed by the Clinical Manager.
• Collaborate with physicians to formulate the initial treatment plan based on their orders and the initial patient assessment. Deliver hands-on care, manage and evaluate the treatment plan, and educate patients according to physician directives under Clinical Manager's oversight. Adapt the plan in consultation with the physician based on continuous assessments and regulatory requirements.
• Coordinate care efforts involving various healthcare professionals (such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
• Continuously report on patient care, condition, and progress to the attending physician and Clinical Manager.
• Implement the patient care plan with the patient and their family to support optimal outcomes.
• Supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions that align with the current plan of care. Monitor proper documentation completion by home health aides/personal care workers as part of your supervisory duties.
• Facilitate patient discharges after consultation with the physician and Clinical Manager, ensuring all required clinical documentation is completed.
• Prepare comprehensive medical documentation for each patient, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan modifications. Compile visit reports, summaries of patient records, and liaise with other healthcare professionals to deliver optimal care.

Required Experience/Skills
• Diploma, Associate, or Bachelor’s Degree in Nursing.
• Preferred: A minimum of one year of experience in home health care.
• Proficient in OASIS.
• Experience with HomeCare HomeBase (HCHB) is advantageous.
• Current and unrestricted Registered Nurse license.
• Current CPR certification.
• Exceptional organizational and communication skills.
• A valid driver's license, auto insurance, and reliable transportation are mandatory.

Pay Range: $45.00 - $63.00 per visit/unit
$70,500 - $96,900 annual base salary.

Scheduled Weekly Hours: 40

The compensation range reflects a good faith estimate of starting base salary for full-time employment (40 hours/week) at the time of posting. Actual pay may vary based on geographical location and individual skills, knowledge, experience, education, certifications, etc. $77,200 - $106,200 annual salary.
