Odyssey House Louisiana, Inc. is seeking a full-time Registered Nurse for the Community Health Center. The schedule is Monday?Friday 8am?5pm. This role is a grant?funded position by the Hepatitis C Elimination Initiative Pilot. The RN provides and coordinates patient care for participants in the agencys HCV grant program within the Federally Qualified Health Center. The role offers direct clinical support, including monitoring patient progress, facilitating access to HCV treatment, and providing education on prevention, treatment options, and overall liver health. Reporting to the Nurse Manager, the RN develops and implements individualized care plans in collaboration with physicians, social workers, and other healthcare professionals to ensure seamless, high?quality care. The RN also serves as a patient advocate, helping participants navigate barriers to treatment, empowering informed health decisions, and supporting them throughout their HCV care journey. Duties & Responsibilities
• Assess patients conditions, monitor vital signs, administer medications and treatments, and ensure patient safety.
• Develop and implement individualized care plans, collaborating with other healthcare professionals to ensure comprehensive and effective patient care.
• Educate patients and their families about health conditions, treatment options, and preventative care. They also advocate for patients needs and ensure their voices are heard.
• Work closely with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a smooth transition of care.
• Administer medications, including injections, IVs, and oral medications, ensuring accuracy and safety.
• Monitor patients responses to treatment, assess their progress, and adjust care plans as needed.
• Prepare to respond to emergencies, providing immediate care and support to patients in critical situations.
• Stay up to date on the latest advancements in healthcare, participate in continuing education, and maintain professional certifications.
• Document all pertinent health?related information about clients on the patient chart that is done routinely and according to nursing standards of practice.
• Complete all incident reports before the end of the scheduled shift if applicable.
• Maintain HIPAA guidelines.
• Perform other duties as assigned.
Minimum Qualifications
• CPR certification, first aid training, and experience in specific areas like emergency or critical care nursing helpful.
• Ability to work collaboratively in a multidisciplinary setting.
• Proficient in electronic health records.
Preferred Qualifications
• Associates Degree in Nursing, Bachelor of Science in Nursing, Nursing Diploma or equivalent.
