This Registered Nurse - Home-Based Primary Care position is located at the North Battlefield Clinic in Chesapeake, Virginia. Please learn more about the position below, along with the duties. Duties The Home-Based Primary Care Registered Nurse is a professional nurse with a broad-based clinical knowledge appointed to effectively implement health promotion, prevention, and management of patients with acute and chronic illness and those that are at the end of life and understand changes associated with aging and principles of growth and development relevant to the adult and geriatric population. HBPC RN is a team member who provides comprehensive and coordinated nursing care to an assigned panel of patients. Duties include, but are not limited to : -The RN collaborates with both VA services and community services, as appropriate, to effectively meet the health promotion or disease prevention, acute, chronic, and long-term needs, based on the Veteran's goals and plan of care with a focus on self-management. -RN implements a continuous process for performance improvement that utilizes data to track and evaluate services and outcomes. -The RN also has the responsibility and accountability to assist in delivering direct nursing care in the home setting. -Interacts, coordinate and advocates for the patient/caregiver with other relevant parties across a continuum of care, beyond a single episode of care, ensuring seamless transitions. -Provides individualized education and support to the patient and caregiver to optimize clinical outcomes and support quality of life decisions. -Provides intensive care management. -Links Veterans with resources. Coordinates resources for comprehensive care to avoid duplication of services and streamline staff interaction and services with the Veteran. -Participates in the development of the individual plan of care at interdisciplinary care meetings for assigned panel of patients.
