Role Overview:

As a Registered Nurse in Home Care, you will deliver and oversee nursing care to patients in their homes, following physician directives, and adhering to all relevant laws and agency policies. You’ll play a vital role in coordinating a comprehensive care plan with healthcare professionals to ensure seamless patient support.

Key Responsibilities:
• Clinical Competence: Develops, initiates, implements, and revises individualized care plans in collaboration with physicians and other healthcare team members. Supervises home health aides and LPNs/LVNs, instructing and assigning tasks per regulations.
• Documentation and Care Delivery: Provides high-quality clinical services according to infection control standards and the established care plan. Completes comprehensive assessments timely and accurately updates all patient encounter documentation.
• Quality Assurance: Conducts initial and ongoing nursing evaluations to ensure eligibility for home health services, formulates updated care plans in consultation with the physician, and submits precise documentation on all care activities.
• Team Collaboration: Responds to directives from Clinical and Executive Directors, assists in onboarding new staff, participates in performance improvement initiatives, and engages in on-call rotations as necessary.

Required Qualifications:
• Valid and unrestricted RN license in the state of practice.
• Minimum of 1 year of experience in Home Health.
• Current Driver’s License, vehicle insurance, and dependable transportation.
• Current CPR Certification.
• Commitment to providing care to clients irrespective of their background.

Preferred Qualifications:
• Ability to work independently.
• Strong communication, organizational, and writing skills.

Our competitive pay ranges from $28.27 to $50.48 per hour, influenced by various factors such as experience and location, alongside a comprehensive benefits package including equity stock purchase and 401k contributions, subject to eligibility.
