Responsibilities

As a Registered Nurse in our Home Health division, you will be a vital point of contact for all disciplines involved in patient care. Your key responsibilities will include:
• Performing visits and coordinating client care
• Consulting with physicians and the office about patient care
• Conducting accurate OASIS collection
• Maintaining current medication profiles
• Timely reporting of lab values to physicians
• Participating in weekly case conferences and monthly case manager meetings

Qualifications

To succeed in this role, Registered Nurses (RNs) must meet the following qualifications:
• Current license as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Proven knowledge and skills in current nursing practice
• A valid state driver's license and automobile liability insurance
