Key Responsibilities:
• Conduct comprehensive assessments of patients and their families to determine home care needs.
• Regularly reassess patient nursing needs and adjust care plans as necessary.
• Develop Plans of Care in collaboration with patients and families, integrating therapeutic and rehabilitative nursing actions.
• Educate and counsel patients/families on disease management, self-care techniques, and preventive health measures.
• Monitor patient conditions during home visits and communicate updates to the Clinical Team Manager daily.
• Implement necessary nursing procedures and administer prescribed medications and treatments.
• Assist with patient discharge planning needs as part of the care planning process.
• Coordinate patient care efforts and report life-threatening situations immediately.
• Provide timely information regarding Home Medical Equipment (HME) and supply needs to the CTM.
• Manage care coordination and communication with healthcare teams, ensuring a seamless patient experience.
• Maintain documentation and prepare clinical notes to keep the healthcare team informed.
• Participate in on-call duties and ensure equipment and necessary items are ready for patient care.
• Supervise and evaluate Home Health Aide care and reported activities bi-weekly.
• Adhere to infection control procedures and promote a safe healthcare environment.
Communication Skills:
• Prepare and update clinical notes for physicians as needed.
• Communicate effectively with team members, physicians, and community health personnel to coordinate optimal patient care.
Additional Role Expectations:
• Be available for patients during business hours (8 a.m. - 6 p.m.) and schedule timely visits.
• Maintain punctual and responsible attendance practices.
• Adhere to agency policies, including privacy and confidentiality standards.
• Demonstrate flexibility and positive communication in a team environment.
• Pursue ongoing education to enhance your healthcare knowledge base.

Location: Please note that our office is located in Corpus Christi, TX.
Required Qualifications:
• High School diploma or equivalent.
• Graduate of an accredited nursing school.
• Active RN license in the state of Texas.
• Current CPR certification.
• Minimum of 2 years clinical experience in community home health or medical/surgical settings.
• Familiarity with electronic medical records.
• Comprehensive knowledge of nursing practices and regulations related to home care.
• Strong communication and problem-solving skills.
