Home Health Registered Nurse Responsibilities
• Provide admissions, case management, and follow-up nursing visits for patients in home health care.
• Deliver ongoing care and manage each patient's case, ensuring necessary follow-up as directed by the Clinical Manager.
• Collaborate with physicians to form initial treatment plans based on orders and initial patient assessments.
• Deliver hands-on care, evaluate care plans, and educate patients following physician directives under the supervision of the Clinical Manager. Update care plans in consultation with physicians as needed.
• Coordinate comprehensive care, involving various healthcare providers such as physical therapists, occupational therapists, and home health aides.
• Relay patient care details, conditions, and progress to the physician and Clinical Manager regularly.
• Implement patient care plans collaboratively with the patient and family to reach optimal outcomes.
• Oversee and supervise the work of Home Health Aides and Personal Care Workers, providing written instructions that align with current care plans.
• Monitor documentation completeness by aides as part of leadership duties.
• Discharge patients with the physician and Clinical Manager, ensuring all clinical documentation is finalized.
• Complete thorough medical documentation, including case conferences, patient interactions, medication updates, and alterations in care plans. Prepare reports, update patient records, and consult with other healthcare professionals for optimal patient care.

Required Experience and Skills
• Diploma, Associate, or Bachelor’s Degree in Nursing
• At least one year of nursing experience is preferred
• Strong background in med-surg, ICU, ER, or acute care experience
• Experience in home health is advantageous
• Hold a current and unrestricted Registered Nurse license
• Maintain current CPR certification
• Possess strong organizational and communication abilities
• Valid driver’s license, insurance, and dependable transportation
Compensation
• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 annually base salary
