Registered Nurse / Home Health Agency (MUST HAVE HOME HEALTH EXPERIENCE) in Victorville, CA

JOB RESPONSIBILITIES Patient Care 1. Complete an assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illnesses. 2. Initiates the plan of care and makes necessary revisions as patient status and needs change. 3. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. 4. Counsels the patient and family in meeting nursing and related needs. 5. Provides health care instructions to the patient as appropriate per assessment and plan of care. 6. Prepares clinical notes and updates the primary physician as necessary. 7. Communicates with the physician, DPCS and agency administrator regarding the patient’s needs and reports any changes in the patient’s condition. Administrative Responsibilities 1. Conduct the initial visit with the patient within 24 hours from the moment patient case was accepted. 2. Communicate any scheduling difficulties with the office immediately preventing any patient care delay. 3. Complete comprehensive care as ordered, report any finding in a timely fashion. 4. Complete and turn in all required paperwork and online charting within 72 hours from the day the visit was conducted. 5. Comply with the basic processes and regulations of the company. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting. 2. Current licensure in the State of California, CPR certification and valid driver’s license. 3. One year of home health care experience preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, nursing skills per competency checklist. 5. Experience working with an EMR system (DSL (preferred), SYNERGY, KINSER) Job Types: Travel nursing, Contract, Per diem Pay: $100.00 - $140.00 per hour Benefits: • Flexible schedule Work Location: On the road
