You will be the primary liaison among all healthcare disciplines involved in patient care, overseeing visit schedules and ensuring optimal service. Key responsibilities include: • Performing patient visits and managing client care coordination • Consulting with physicians regarding patient care and reporting essential details • Accurately collecting OASIS data and keeping medication profiles current • Reporting lab results to physicians promptly • Participating in weekly case conferences and monthly case manager meetings Qualifications To be considered for the Registered Nurse position, candidates must meet the following qualifications: • Valid Registered Nurse (RN) license in the state of employment • Preferred minimum of one year of clinical experience • Demonstrated knowledge and skills in current nursing practices • Valid state driver's license and automobile liability insurance Additional Information Enhabit Home Health & Hospice is an equal opportunity employer.
