In this role as a Registered Nurse in Home Health, you will deliver high-quality nursing care to patients in their homes, following physician recommendations and complying with applicable regulations and policies. You will work collaboratively with other healthcare professionals to ensure a seamless and comprehensive plan of care. Your contribution will be crucial in maintaining continuity of patient care through effective communication and coordination with physicians, agency personnel, and community resources.

Key Responsibilities:
• Clinical Competence
• Develop and implement a personalized plan of care in collaboration with the physician and healthcare team.
• Supervise home health aides and licensed practical nurses, providing guidance and training as needed.
• Conduct required visits to ensure quality of care.
• Documentation and Care Delivery
• Deliver exceptional clinical services within the scope of practice and infection control standards, aligning with the care plan.
• Accurately complete comprehensive assessments, including OASIS and medication reconciliation.
• Document patient visits promptly following policy and payer requirements.
• Quality
• Conduct evaluations to determine eligibility for home health and establish medical necessity for care.
• Share relevant patient information effectively with agency staff, ensuring continuity of care.
• Communicate proactively with physicians, patients, and families to uphold standards of quality care.
• Teamwork
• Follow the direction of Clinical and Executive Directors and complete tasks within designated timelines.
• Assist in orienting new staff and mentor peers and students.
• Engage in performance improvement activities to enhance patient outcomes.
• Participate in on-call and weekend rotations as needed.
• Contribute to the agency's utilization management and strive for optimal patient care practices.

Requirements:
• Current and unrestricted RN license in your state of practice.
• Minimum of 1 year of Home Health experience.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.
• Current CPR Certification.
• Ability to provide care to clients regardless of their background or circumstances.

Preferred Qualifications:
• Proficiency in working independently.
• Strong communication, writing, and organizational skills.

Compensation will be determined based on various factors, including local job markets, experience, and educational background. In addition to a competitive hourly wage ranging from $28.27 to $50.48, we provide a comprehensive benefits package, recognition programs, and a 401(k) with company contributions (subject to eligibility requirements).
