Registered Nurse, Home Health
Make a difference every day as a nurse with CenterWell Home Health. Deliver individualized, one-on-one care to help patients recover and regain independence in their homes. Collaborate with a dedicated team of physicians and healthcare professionals to create and manage care plans that enhance recovery and help patients return to their cherished activities.
As a Home Health Registered Nurse, you will:
• Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management, and provide necessary follow-up as directed.
• Collaborate with physicians to develop initial treatment plans based on orders and assessments. Offer hands-on care, manage care plans, and provide patient education under supervision.
• Adjust care plans in collaboration with physicians as needed.
• Coordinate comprehensive care involving a range of healthcare professionals.
• Report on patient care, condition, and progress to physicians and management.
• Develop patient care plans with patient and family input to meet needs effectively.
• Supervise and direct the work of home health aides and other care staff, and ensure proper documentation is completed.
• Discharge patients after consultation, completing necessary clinical documentation.
• Prepare and maintain medical documentation, including changes and updates to the care plan.
Utilize Your Expertise to Make a Difference
Required Experience/Skills:
• Diploma, Associate, or Bachelor Degree in Nursing
• At least one year of nursing experience preferred
• Strong experience in med surg, ICU, ER, or acute settings
• Experience in Home Health is advantageous
• Valid and current Registered Nurse license
• Current CPR certification
• Strong organizational and communication abilities
• Valid driver's license, auto insurance, and reliable transport
Compensation Range
• $45.00 - $63.00 per visit/unit
• $70,500 - $96,900 annually
Weekly Hours

The compensation range below is a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. It may vary based on individual qualifications, skills, and location.
• $77,200 - $106,200 annually
