
Registered Nurse - Home Health in Caledonia, OH
Job Description
As a Registered Nurse, you will play a vital role in delivering and directing nursing care to patients at home, following physician prescriptions and abiding by all applicable laws and agency policies. You will work collaboratively with a team of healthcare professionals to ensure continuity of care and effective communication among patients, families, and community resources.
Key Responsibilities:
• Deliver high-quality clinical services while adhering to scope of practice and infection control guidelines.
• Coordinate care with all members of the patient’s healthcare team, from admission through discharge.
• Conduct comprehensive clinical nursing assessments as per federal/state requirements and payer needs.
• Confirm patient eligibility and medical necessity for services defined by insurance and agency policies.
• Develop and update individualized care plans in collaboration with other healthcare providers.
• Adjust frequency and duration of care plans as needed, with physician approval.

Required Qualifications:
• Current and unrestricted RN licensure in the relevant state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
• Ability to work effectively in diverse home environments, regardless of client background.
Preferred Qualifications:
• At least 1 year of RN experience.
• Ability to work independently.
• Excellent communication, writing, and organizational skills.
Your compensation will depend on various factors, including local labor markets and your credentials. We offer a comprehensive benefits package, including incentive and recognition programs, equity stock purchase options, and 401k contributions (subject to eligibility). The hourly pay for this role is between $28.27 and $50.48 based on full-time employment.
