Registered Nurse (Home Health) - Rockville, MD
Location: Rockville, MD
Job Type: Full-Time
Work Setting: Home Health / Residential
Schedule : Day and Night shifts available (Weekends may be required)
Patient Population : Pediatric and adult patients (Age range: 0-75 years)
Job Overview
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, patient-centered care in a home health setting . This role involves caring for both pediatric and geriatric patients , performing comprehensive assessments, implementing care plans, and delivering skilled nursing services in patients' homes. The ideal candidate is clinically strong, detail-oriented, and committed to improving patient outcomes within the community.
Key Responsibilities
• Perform comprehensive patient assessments and ongoing evaluations
• Develop, implement, and update individualized care plans in collaboration with the healthcare team
• Administer medications and prescribed treatments safely and accurately
• Provide skilled nursing care including:
• Tracheostomy care
• Ventilator management
• G-tube and J-tube feeding and care
• Monitor patient progress and promptly respond to changes in condition
• Educate patients and family members on health management, treatments, and care techniques
• Deliver pain management interventions as appropriate
• Maintain accurate, timely, and compliant patient documentation and charting
• Coordinate care with physicians, therapists, and other healthcare professionals
• Ensure adherence to all clinical, safety, and regulatory standards
Requirements
• Active Registered Nurse (RN) license in the state of practice
• CPR certification (required)
• Minimum 3 years of nursing experience preferred
• New graduates may be considered
• Prior home health experience strongly preferred
• Pediatric home health experience is a plus
• Hands-on experience with:
• Tracheostomy care
• Ventilators
• G & J feeding tubes
• Strong clinical judgment and patient assessment skills
• Ability to work independently in home-based settings
• Ability to manage multiple priorities in a fast-paced environment
• Must pass background and compliance screenings
