
Registered Nurse - Home Health Services in Ashdown, AR
Job Description
As a Registered Nurse in Home Health, you will play a vital role in delivering and overseeing nursing care to patients in the comfort of their homes, as directed by physicians and in line with relevant laws, regulations, and agency policies. You will coordinate care plans with other healthcare professionals, ensuring a seamless continuum of care by effectively communicating with physicians, agency staff, patients, families, and community resources.
Key Responsibilities:
• Clinical Competence: Initiate, develop, implement, and adjust care plans in collaboration with physicians and other healthcare professionals. Supervise home health aides and LPNs/VNs, providing guidance and assigning tasks per state and federal regulations, while ensuring all supervisory visits are conducted as required.
• Documentation and Care Delivery: Deliver high-quality clinical services within your scope of practice and infection control standards, adhering to the care plan and collaborating with the healthcare team. Complete comprehensive assessments (OASIS), including medication reconciliation, accurately and promptly. Document all patient visits according to policy and payer requirements, syncing timely per LHC guidelines.
• Quality: Conduct initial and comprehensive nursing evaluations, verifying patient eligibility and medical necessity for home health services. Communicate pertinent information effectively among agency staff regarding patient care, visit assignments, and other essential data.
• Teamwork: Receive direction from the Clinical Director and Executive Director professionally, ensuring timely completion of assigned tasks and required learning. Assist in orienting new personnel and serve as a preceptor for other staff and students. Actively participate in performance improvement initiatives and contribute to surveys for quality assurance.

Required Qualifications:
• Current and unrestricted RN licensure.
• 1+ years of experience in Home Health.
• Valid Driver’s License, vehicle insurance, and access to reliable transportation.
• Current CPR Certification.
• Ability to adapt to diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications:
• Capability to work independently.
• Strong communication, writing, and organizational skills.
Pay is determined by several factors, including local labor markets and experience. We offer a comprehensive benefits package, incentive programs, equity stock purchase, and 401k contributions. Hourly pay ranges from $28.27 to $50.48 for full-time positions. We comply with all applicable minimum wage laws.
