Your Role as a Hospice RN / Case Manager: • Serve as the primary contact for patients and their families, guiding them through every step of their hospice journey. • Deliver skilled nursing care based on individual Plans of Care in homes, assisted living facilities, or any environment they call home. • Conduct initial and ongoing assessments to monitor patient conditions and adjust care as necessary. • Work collaboratively with a diverse interdisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains to create tailored care plans. • Provide emotional, physical, and spiritual support to patients and their families, offering education and guidance through the bereavement process. • Instruct caregivers on safe and confident ways to care for their loved ones, offering practical training and emotional support. • Communicate essential updates regarding patient conditions to the care team for timely interventions. • Maintain accurate clinical documentation and adhere to infection control and compliance standards. • Contribute to organizational quality improvement and scheduling initiatives to ensure consistent and compassionate care. • Assist patients and families in understanding critical topics such as medication administration, hospice philosophy and services, symptom and pain management, and end-of-life expectations. What We Seek from You: • Active RN license in the state of employment (or eligibility to obtain). • At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields. • A sincere hospice heart: compassionate, empathetic, and patient-centered. • Comfortable providing care in various settings, including private homes and facilities. • Excellent communication and critical thinking skills in emotionally sensitive situations. • Valid driver's license, auto insurance, and reliable transportation. • CPR certification required. Preferred Experience (Not Required): • Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Background in admissions, wound care, float pool, or travel nursing. • Experience with terminally ill patients or serious illness support. Compensation and Benefits: We offer a competitive salary range of $84,490 - $99,340, which may vary based on factors such as location, experience, and education.
