The Impact You'll Make (Your Core Responsibilities) As a Berkley Hospice RN Case Manager, you are the champion of the patient experience. You will manage a meaningful caseload (home and facility-based) by: • Clinical Leadership: Performing expert care coordination, advanced symptom management, ongoing assessment, and complex clinical care. • Patient Advocacy: Serving as the key contact to ensure patient comfort, dignity, and a high quality of life. • Relationship Building: Establishing strong, trusting bonds with patients, families, and the Interdisciplinary Group (IDG) team. • Collaborative Care: Partnering closely with our dedicated social workers, chaplains, certified aides, and clinical leadership to deliver holistic support. What We Need From You (Qualifications) We are looking for a Heart-Centered RN ready to invest in our mission: • Active, unencumbered Colorado RN license. • Minimum of 1-2 years of clinical experience; Hospice, Palliative Care, or Home Health experience is highly preferred. • Dependable, adaptable, and a skilled communicator who thrives in a team-based setting. • Valid driver's license and reliable transportation for travel within the South Metro service area.
