Your Role: • Evaluate and assess patients to determine their eligibility for hospice care. • Develop individualized care plans tailored to each patient’s needs, utilizing available resources effectively. • Communicate seamlessly with the care team, physicians, payors, patients, and families to address and fulfill care requirements. • Deliver high-quality skilled nursing care and oversee the activities of other care team members. • Document all care delivery meticulously in our electronic medical records (EMR) system. • Foster a culture of accountability and teamwork, contributing positively to the work environment.
