Park Nicollet Hospice is looking to hire a Referral and Scheduling Registered Nurse to join our team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The Referral and Scheduling RN is essential to the seamless coordination of referrals and scheduling across Home and Community Services, supporting a broad range of programs including hospice, home care, palliative care, and other specialized community services. This role manages the referral through admission, and scheduling processes ensuring patient visits are scheduled appropriately to maintain manageable caseloads and high-quality care. The RN collaborates closely with interdisciplinary teams, coordinates coverage when team members are unavailable or require visit support, and serves as a key resource for clinical, reimbursement, and regulatory matters. Compassionate communication, strong organizational skills, and a commitment to supporting patients, families, and staff are central to success in this role.
This role will focus primarily on supporting Hospice referral and scheduling.
Work Schedule: Monday – Friday 8a-4:30p. Q 4th-6th weekend
Required Qualifications:
Education, Experience or Equivalent Combination:
Bachelor’s degree in nursing
2+ years of hospice, palliative care, home care, and other community care service experience
Licensure/ Registration/ Certification:
MN or WI Registered Nurse
The organization pays the renewal fees for the second license
Knowledge, Skills, and Abilities:
Excellent verbal and written communication skills.

Strong computer and technology skills
Preferred Qualifications:
Education, Experience or Equivalent Combination:
5+ years hospice, palliative care, home care, and other community care service experience
Licensure/ Registration/ Certification:
MN and WI RN Licensure
Knowledge, Skills, and Abilities:
Experience with the EPIC EMR setting
