About the Role: As a Registered Nurse in Home Health, you will deliver compassionate care to patients in the comfort of their homes, following physician directives and abiding by all relevant laws and agency regulations. Your role will involve coordinating care with other healthcare professionals to ensure seamless quality of care.
Your Key Responsibilities:
• Clinical Expertise: Develop and implement patient care plans in collaboration with physicians and healthcare teams.
• Supervision: Oversee the activities of home health aides and licensed practical nurses while ensuring compliance with state and federal regulations.
• Quality Care Delivery: Execute high-quality services aligned with care plans and infection control protocols.
• Documentation: Conduct comprehensive assessments (OASIS), accurately documenting patient interactions in accordance with policy and payer requirements.
• Communication: Effectively relay important information regarding patient care issues and collaborate with physicians and family members for optimal outcomes.
• Team Collaboration: Work closely with the Clinical Director and Executive Director, assisting in onboarding new staff and participating in performance improvement initiatives.

Qualifications You’ll Need:
• Current, unrestricted RN licensure.
• Minimum of 1 year of Home Health experience.
• Valid Driver's License and dependable transportation.
• Current CPR Certification.
• Ability to provide care in diverse home settings, respecting all aspects of patient identity.
