Intake Nurse Responsibilities:
• Serve as a liaison with referral sources, agencies, health professionals, and field staff.
• Manage all incoming calls, providing essential information to help patients and referral sources access services.
• Communicate with patients and families to facilitate the initiation of care/services.
• Conduct intake screenings, documenting and analyzing all required information for preliminary admission decisions.
• Log referrals and prepare reports, providing responsive telephonic contact and on-call support as needed.
• Actively promote care/services to potential patients and clients.
• Receive verbal orders from physicians for care/services where permitted by law.
• Collect and input customer information into the management information system.

Essential Requirements for Intake Nurses:
• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) licensure in Michigan.
• At least one year of experience as an RN.
• Home health care experience preferred but not required.
