Overview:
Join Our Team as a Registered Nurse - Integration
Is it important to you that your patients and their families know and feel that you are with them? Do you have strong interpersonal skills and know how to foster a positive and engaging work environment? We are looking for registered nurses - integration who are committed to creating meaningful patient experiences.
In this role, you'll be responsible for supporting the integration team with compassionate patient care at de novo locations across the region. You'll work collaboratively with the care team and assist with training the staff of the acquired location. This role requires extensive travel and on-call responsibilities. Most importantly, you will help ensure all patients are receiving consistent quality care and that care team members have the support they need to effectively manage the transition.

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse (compact/multi-state license required)
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience required)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
