As a Registered Nurse – Nurse Care Coordinator, you will be responsible for coordinating patient care, facilitating communication between healthcare providers, and ensuring that patients receive the necessary resources and support to achieve optimal health outcomes. You will work closely with our interdisciplinary team to develop and implement individualized care plans, conduct discharge planning, and provide education and support to patients and their families.
• *Key Responsibilities:**

• Coordinate patient care and services, including hospital admissions, discharges, and transfers

• Develop and implement individualized care plans, in collaboration with the interdisciplinary team

• Facilitate communication between healthcare providers, patients, and families to ensure seamless care transitions

• Conduct discharge planning and ensure that patients receive necessary resources and support

• Provide education and support to patients and their families on disease management, self-care, and health promotion

• Collaborate with healthcare providers to develop and implement care coordination strategies

• Monitor and report patient outcomes, and identify opportunities for quality improvement

• Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines
• *Requirements:**

• Current Montana licensure as a Registered Nurse

• Bachelor of Science in Nursing degree

• Three (3) years of experience in a high-volume case management program in a hospital or clinic setting

• Strong leadership, management, and communication skills

• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities

• Proficiency in electronic medical records (EMR) and other standard office equipment and computer applications
