Registered Nurse - Office - Primary Care - Brewer
Provide nursing care in physician practices, duties can vary by practice specialty. Carry out instructions provided by clinical coordinator(s), clinical supervisors and Director, Clinical Services of Physician Practices.
Responsibilities include:

• Adheres to established organization policies and procedures to include, but not limited to HIPAA regulations, Code of Conduct, etc., as per IDD/PCD policies.
• Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with the basic principles and guidelines of professional nursing.
• Reports variance from expected outcomes and communicates changes in patient's conditions to the provider in a timely manner.
• Mandatory in-services - 100% compliance of iCARE (complete annually at least 7 days prior to evaluation). Employee Updates and any other in-services/meetings and training as assigned.
• Maintains RN licensure in accordance with Maine State Board of Nursing. Maintains current BCLS certification.
• Participates in multidisciplinary committee work that relates to unit practice.
• Responsible for completing clinical staff competency training.
• Performs and documents results of point of care testing and appropriate interventions.
• In-service/teaches staff about new policies/procedures and patient care standards if requested.
• Demonstrates correct technique in the use of equipment according to specific product information and policy and procedure manuals.
• Follows appropriate procedure for obtaining and returning/cleaning/disposing of equipment and supplies.
• Is responsible for unit cleanliness and maintaining a neat and safe environment.
• Administers and documents medications according to nursing policy and procedure following the 5 Rights: Right Patient. Right Medication. Right Time. Right Route. Right Dose.
• Immunizations are scheduled and administered according to practice guidelines and documented.
• Performs and documents results of blood glucose monitoring and appropriate interventions.
• Ensures sterile technique(s) are maintained. Patient and room are ready when the provider arrives. Consent is signed and available. Time out performed if required.
• Room and supplies are processed according to Infection Control policies.
• Specimens are labeled in front of the patient and processed per policy.
• Appropriate patient education occurs post procedure.
• Patients wait 20 minutes after medical administration at which time patient's condition is documented. Patient is then discharged from the office.
• Medication errors are reported to provider and supervision immediately.
• Adverse reactions are referred to provider ASAP - do ADR.
• Will provide telephone triage services as needed.
• Demonstrates an ongoing awareness of and participation in the Quality Assessment and Improvement Program.
• Assist in the development and implementation of patient care standards.
• Patient's fee ticket is reviewed before the patient leaves the clinic.
• All medications and treatments are documented on fee ticket.
• Finishes own work on time, 95% of the time, avoiding overtime.
• Seeks and engages in opportunities for personal and professional growth and development.
• Demonstrates excellent interpersonal and communication skills.
• Demonstrates the ability to manage conflict.
• Attends monthly staff meetings and/or reads and initials meeting minutes.
• Collaborates with members of the health care team and the patient/family.
• Effectively facilitates communication between the physician and others in order to achieve established goals for the patient.
• Communicates decision related to clinical and operational roadblocks to patient/family, physicians and members of the healthcare team.
• Screens referrals from providers on complex situations that may require outside resource intervention.
• Demonstrates the ability to develop relationships across organization lines using interpersonal communication skills.
• Obtains CEU's pertinent to area of practice in collaboration with the Lead Physician and Director/Office Manager. Has documented evidence of continuing education appropriate to level practice.
• Assumes responsibility for own personal growth.
• Responds to all patient calls, walk-ins that require triage.
• Available to providers, clinical and clerical staff to assess any unstable patient needs.
• Collects, reviews and documents patient's current health status and history according to standard. Counsels' patients to care for or cope with his/her condition. Provides instruction for self-care management of acute or chronic medical conditions.
• Provide effective assessment of patient's needs, identifying and addressing needs as indicated.
• Assists patients/families to understand and comply with their established plan of care.
• Accepts telephone orders within scope of practice or delegates when appropriate and documents and reads back the order.
• Demonstrates dedication to the pursuit of excellence in improving the health care of the patient.
• Initiates care management referrals for patients meeting care management criteria as indicated.
• Demonstrates the ability to organize and carry out duties without direct supervision.
• Demonstrates the ability to problem solve using clinical and critical thinkings skills.
• Demonstrates the ability to identify the community needs of the patient and refer as indicated.
• Seeks and maintains knowledge of community services/resources, support programs, different levels of care services to include, but not limited to home health services, long-term care, skilled level of care, free/reduced drug programs and state and federal programs.
• Assist with patient flow through the Clinic.
• STAT orders are handled first.
• Checks completion of all orders at the end of shift or delegates.
• Appropriate patient teaching (explanation of test results, medication side effects, medication reactions and dosages.
• Follow up visits are scheduled at time of discharge per protocol and Patient/Family, etc. is notified of next appointment.
• Physicians are assisted with admissions: Call Registration, call report to the Unit, photocopy all practice documentation to go with the patient, arrange transport. Completes nursing assessment form and sends with patient.
• Assists with minor surgical procedures.
• Recognizes and responds appropriately to emergency situations including making appropriate calls, directing traffic and obtaining equipment.
• Schedules are kept current: Schedules are prepared in advance of patient days (or delegated) and updated at least twice per session to keep provider(s) informed of schedule.
• Utilizes skill in identifying processes that enhance delivery of service excellence to customers and appropriate disseminates that information.
• Assist with updating practice policies and procedures.
• Provides patient education appropriate to age and literacy level. Provide patients and/or family, etc., with education appropriate to identified needs according to standard and documents into the patient's record. Educational handouts are given in advance of the visit as requested by the provider (i.e., well child packets).
• Documents patient readiness to learn and barriers to learning.
• Counsels' patients to care for or cope with his/her condition.
• Anticipates patients' needs and initiates orders per protocol for provider signature.
• Duties are adequately performed as needed or seeks out supervisory guidelines.
