Responsibilities: • Implement and monitor effective patient care plans, ensuring accurate documentation and communication of patient conditions. • Act as the primary coordinator for multidisciplinary teams to facilitate seamless patient care. • Efficiently execute physician and nursing orders while vigilantly monitoring patient progress. • Evaluate and coordinate discharge planning needs in close collaboration with the healthcare team. • Utilize expertise to assess patients' physical, psychosocial, cultural, spiritual, and age-related needs, delivering personalized and compassionate care. • Delegate appropriate care tasks to nursing personnel, optimizing both patient care and team performance. • Engage in daily interdisciplinary patient rounds, demonstrating comprehensive knowledge of patients' conditions to achieve optimal outcomes. • Adhere to the ANA Scope and Standards of Nursing Practice and the ANA Code of Ethics. • Develop, implement, and adjust individualized care plans tailored to meet each patient's unique needs. • Participate in ongoing education activities to enhance your professional growth and nursing practice. Qualifications: • Active RN license in Connecticut. • Graduate from an accredited nursing program; a BSN is preferred. • Current certifications in BLS, ACLS, and PALS. • Experience in PACU, ICU, or ED settings is preferred.
