
Registered Nurse Patient Care Leader in Brownsville, KY
Job Description
As the Patient Care Leader, you will oversee and coordinate exceptional clinical services. You will lead an interdisciplinary team, ensuring high-quality care for home health patients in accordance with physician directives and applicable state and federal regulations.
Key Responsibilities:
• Deliver clinical services in accordance with state nursing practice guidelines and the individualized plan of care.
• Manage patient referrals, assigning clinicians appropriately, and schedule start-of-care visits.
• Determine patient home health benefits, assess medical necessity, and secure ongoing insurance approvals.
• Continually assess patient needs and deliver customized care that meets eligibility criteria and physician orders.
• Review assessments and care plans daily, and collaborate with clinicians to enhance patient outcomes.

Required Qualifications:
• Current, unrestricted RN license in your practice state.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and reliable transportation or public transport access.
• Ability to work effectively in any home situation, regardless of client demographics or circumstances.
Preferred Qualifications:
• Experience in home care settings.
• Strong ability to work independently.
• Excellent communication, writing, and organizational capabilities.
Your compensation will be competitive, ranging from $71,200 to $127,200 annually, depending on factors like location, qualifications, and experience.
