Key Responsibilities:
As a Case Manager RN, you will provide clinically-based case management to ensure effective patient care delivery. Responsibilities include:
• Utilizing management, care coordination, and transition planning.
• Overseeing a designated case load and planning to meet patient needs efficiently.
• Collaborating with the interdisciplinary team to support physicians in optimizing patient care.
• Enhancing quality outcomes and patient satisfaction while managing care costs.
• Providing timely and accurate information to payers.
This position includes a Northern New Mexico differential of $4.00 per hour.
Qualifications:
• Current NM State or Multistate License.
• BLS certification required at time of hire.
• For Level I: ADN.
• For Level II: ADN + 5 years of case management experience or BSN.
• For Level III: MSN or BSN + 5 years of case management experience.
• For Level IV: MSN + 5 years of experience or BSN + 10 years of experience.
