A Brief Overview As a Transfer Center Coordinator, you will be the vital link for transferring patients to the best-suited Avera hospital within our network. Your role is crucial in understanding each patient's needs and ensuring they receive timely and appropriate placements. Collaborating with care team members, nursing departments, and physicians, you will facilitate the seamless movement of patients. You will also gather essential clinical data to support continuity of care and proactively address any potential delays in treatment. What You Will Do: • Manage patient flow from outlying facilities and clinics to create an efficient and seamless entry process. • Utilize critical thinking skills to prioritize and place patients in suitable regional sites, in line with department policies and guidelines. • Serve as a liaison with the bed placement coordinator to ensure timely patient transfers. • Assess the transfer center process and strive for optimal outcomes. • Collaborate with physicians, nurses, and healthcare providers to assess patient placement effectively. • Incorporate research and evidence-based practices in your approach. • Integrate healthcare technology and information management systems into daily responsibilities. Essential Qualifications • Current Registered Nurse (RN) license active in your state of practice upon hire. Preferred Qualifications: • 1-3 years of nursing experience.
