
Registered Nurse - Per Diem in Arenas Valley, NM
Job Description
Join one of the largest Home Health Care companies in New Mexico! Ambercare Homecare is seeking a dedicated Registered Nurse to join our fast-growing team in Silver City, NM.
Position: Per Diem (PRN)
Schedule: 2 weekends a month

Your Role:
• Conduct initial assessments of patients and families to identify home care needs.
• Monitor and document any changes in patient conditions, and promptly notify the supervisor.
• Regularly reassess patient nursing needs and adjust care plans accordingly.
• Develop comprehensive care plans that include therapeutic, preventive, and rehabilitative nursing actions, involving the patient and family in the process.
• Administer medications and provide direct patient care as laid out in the State Nurse Practice Act.
• Counsel patients and families to meet nursing and healthcare-related needs.
• Provide appropriate healthcare instructions based on assessments and care plans.
• Identify discharge planning needs and implement them before patient discharge.
• Coordinate patient care as a Case Manager when assigned by the Clinical Supervisor.
• Prepare clinical notes and ensure that the primary provider is updated at least every 60 days.
• Communicate effectively with healthcare providers regarding patient needs and changes in conditions.
• Work with community health professionals to coordinate care plans.
• Perform additional duties as assigned.
Qualifications:
• Graduate of an accredited registered nursing program and licensed in the state.
• At least one (1) year of clinical nursing experience; preference for intensive care, rehabilitation, or medical-surgical nursing.
• Home health experience is a plus.
• Current CPR certification is required.
• Must have reliable transportation, a valid driver's license, and appropriate automobile insurance.
