Role Overview - What You Will Do as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, guiding them through their hospice experience.
• Administer and document skilled nursing care tailored to each patient's individualized Plan of Care in their home or preferred setting.
• Conduct initial and ongoing assessments to monitor and adjust patient care as necessary.
• Work collaboratively with a multidisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to create and enhance personalized care plans.
• Provide emotional, physical, and spiritual support to both patients and their families, offering education, comfort, and bereavement assistance.
• Train caregivers to ensure they can provide safe and confident care to their loved ones.
• Relay significant updates regarding patient conditions to the care team for timely and appropriate responses.
• Maintain accurate clinical documentation while adhering to infection control and compliance standards.
• Support quality improvement initiatives and scheduling efforts to guarantee compassionate and efficient care.
• Assist patients and families in navigating discussions about medication administration, hospice philosophy, symptom management, and end-of-life expectations.
Qualifications - What You Bring to the Team:
• Active RN license in the state of employment (or eligible to obtain).
• 1+ years of RN experience in hospice, home health, ICU, oncology, geriatrics, or related fields.
• A genuine compassion for hospice care: empathetic and patient-centered.
• Experience providing care in various environments including private homes and facilities.
• Strong communication and critical thinking abilities, particularly in emotionally challenging situations.
• Valid driver's license, auto insurance, and reliable transportation required.
• CPR certification needed.
Preferred Experience (Not Mandatory):
• Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
• Admissions RN, wound care, float pool, travel nursing
• Experience with terminally ill patients or serious illness support
