Job Description

Promotes individual(s) health goals by completing the nursing process for assessment. Collaborates with physicians and IDT. Identifies individual care needs, documents care services in individuals EHR. Maintains safety, asepsis, and ensures the use of universal precautions are followed . Provides quality care by adhering to therapeutic standards and measures health outcomes against individual care goals. Is available in person and remotely for triage and provides clinical support to unlicensed personnel as needed. Ensure continuity in care. Must hold an unencumbered license for NJ State issued by the New Jersey Board of Nursing as a registered nurse. Background in home health, developmental disabilities or mental health preferred. escalate concerns to prescribing providers • Provide guidance on medication administration processes (within scope and per program policy), including safe storage, documentation, and observation for adverse effects. • Monitor and provide oversight of health-related treatments and protocols (e.g., wound care, enteral feeding, oxygen, glucose monitoring) as permitted by policy and licensure. • Identify and document health and safety risks; recommend mitigation strategies and supports to reduce preventable incidents • Provide consultation on infection prevention practices and outbreak response per policy. • Ensure appropriate reporting and follow-up for incidents, sentinel events, and mandated reports in accordance with program requirements. • Provide health education and coaching to individuals, families/guardians, and direct support staff on • Chronic disease self-management strategies • Recognizing signs/symptoms requiring medical attention • Medication side effects and adherence supports • Nutrition/hydration, skin integrity, and general wellness • Assist in developing and delivering training related to health plans and nursing-related supports as required • Maintain timely, accurate clinical documentation in designated systems consistent with nursing standards and program requirements. • Ensure confidentiality and compliance with HIPAA and applicable state and agency policies. • Contribute to audits, quality improvement activities, and regulatory reviews as needed. • Ability to lift, transfer, or assist individuals with mobility. • Flexible schedule including evenings, weekends, and holidays as needed. • Work may take place in residential homes, day programs, or community settings. • Required to visit and support individuals during hospital stays to ensure quality care and communication with medical teams Competencies • Comprehensive clinical assessment and care planning Conducts thorough nursing assessments, identifies health risks, and develops clear, actionable nursing recommendations that align with each individual’s medical needs, safety considerations, and person‑centered support goals. • Care coordination and interdisciplinary collaboration Communicates and collaborates effectively with physicians, specialists, behavioral health providers, therapists, hospitals, and the interdisciplinary team to ensure continuity of care, timely follow‑up, and well‑coordinated service delivery. • Health monitoring and clinical intervention Monitors changes in medical status, recognizes early warning signs, and initiates appropriate interventions, referrals, and risk‑mitigation strategies to prevent complications and maintain optimal health outcomes. • Medication oversight and safe practice Reviews medication regimens for appropriateness and safety, identifies potential interactions and adherence challenges, and provides guidance on medication administration, storage, documentation, and monitoring for adverse effects. • Documentation and regulatory compliance Completes all clinical documentation—including assessments, nursing notes, incident follow‑ups, ISP/POC contributions, and health‑related reports—accurately, thoroughly, and in alignment with nursing standards, HIPAA, state requirements, and agency policies. • Health education and coaching Provides clear, accessible education to individuals, families/guardians, and direct support staff on chronic disease management, recognizing medical concerns, medication safety, wellness practices, and other essential health topics. • Clinical leadership and staff support Serves as a resource to unlicensed personnel by offering guidance, triage support, on‑site and remote clinical consultation, and training related to health plans, risk management, and nursing‑related supports. • Infection prevention and safety management Applies infection control principles, universal precautions, and outbreak response practices consistently while identifying health and safety risks and recommending strategies to reduce preventable incidents. • Emergency response and critical decision‑making Responds effectively to medical emergencies, uses sound clinical judgment in crisis situations, and ensures appropriate reporting, follow‑up, and communication with medical teams. • Adaptability and problem solving Adjusts to changing health needs, transitions of care, shifting schedules, and varied work environments (residential, day program, community, and hospital settings) while maintaining high‑quality nursing support. • Mission‑driven professional conduct Demonstrates commitment to the organization’s mission by promoting dignity, person‑centered care, health equity, and compassionate support for individuals with intellectual and developmental disabilities across all interactions.
