Registered Nurse (RN) - Primary Care / Community Health
Schedule
• Monday-Friday, 8:30 AM - 5:00 PM
• One evening shift per week (11:00 AM - 7:30/8:00 PM on Monday or Tuesday)
• Saturday rotation once every 1-2 months
Position Overview
The Registered Nurse (RN) provides direct patient care in a primary care/community health setting, supporting patients both in person and by phone. This role includes triage, assessment, medication management, immunizations, and patient education, working collaboratively with providers and the care team to deliver high-quality, patient-centered care.
Key Responsibilities
• Assess patients' basic health status and care needs for scheduled and unscheduled visits, including telephone triage
• Analyze clinical data and make informed nursing judgments related to patient care
• Evaluate outcomes of nursing interventions and update care plans as needed
• Collaborate with primary care providers, triage nurses, and interdisciplinary care team members
• Review and process medication refill requests from pharmacies and patients per established protocols
• Administer immunizations and medications
• Perform clinical procedures including EKGs, suture removal, wound care, ear irrigations, PPD placement/reading, and blood pressure checks
• Provide health education to patients and families to support self-management and wellness
• Support a safe, efficient, and compassionate care environment
Required Qualifications
• 1-2 years of direct patient care experience as a Registered Nurse
• Active MA RN license
• Active BLS certification
• Experience using EPIC
Preferred Qualifications
• Prior experience in primary care or community health
• Bilingual in Spanish and/or Portuguese
• Experience working with underserved populations
