Job Description

Summary/Objective The Staff Registered Nurse (RN) provides nursing services to youth residing in the Psychiatric Residential Treatment Facility (PRTF). This role reports to the Lead RN and ensures the delivery of high-quality, trauma-informed care that meets each youth’s medical and psychiatric needs in alignment with Boundless policies, the OhioRISE PRTF Provider Manual, and applicable state and federal regulations. The RN is responsible for working within the scope of their license to assess, monitor, document, and support the physical and behavioral health of youth. The RN collaborates with interdisciplinary teams, families, and outside providers to coordinate care, engage in discharge planning, and uphold compliance with all PRTF licensing requirements. The RN is an essential member of the treatment team supporting youth through healing and recovery. RN will also be responsible for delegating nursing tasks to certified direct service providers. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Provides comprehensive nursing care to youth in the PRTF, including health screenings, vital signs, response to changes in condition, and behavioral crisis support. Maintains first-aid supplies and provides first- aid treatment as necessary; screens for communicable and nuisance diseases. • Ensures trauma-informed, culturally responsive care is consistently delivered. • Monitors and documents youth health status, treatment progress, and responses to interventions; communicates findings to the Lead RN and appropriate team members. • Coordinates medical and wellness services with providers inside and outside of Boundless. • Provides medical and wellness input to interdisciplinary teams and assigned committees related to strategic planning, policy development, and quality assurance. • Participates in treatment team meetings, ensuring medical information is incorporated into Individual Service Plans (ISP). • Completes timely documentation in accordance with OhioRISE and Medicaid requirements, including nursing notes, medication logs, incident reports, and discharge planning summaries. • Assists in the discharge process, ensuring follow-up care, medications, and supplies are arranged and that families/providers are appropriately informed. • Delegates tasks to and provides oversight of direct support staff in accordance with scope-of-practice and state regulations. Maintains medication equipment and supplies; performs skilled nursing procedures (e.g., applies and changes dressings, catharizes); requests, secures and follows up on medical information of residents. • Participates in the development of Plans of Correction when required • Provides and documents health-related training and support to staff as appropriate. • Follows the Individual Service Plan(s) which includes but is not limited to use of mechanical lifting devices and behavioral interventions. • Attends required meetings and ongoing training to maintain competencies and certifications. • Adheres to Boundless policies and procedures and complies with all applicable laws, regulations, licensure, and accreditation standards. • Participates in on-call rotation as assigned. • Performs other related duties as assigned. Minimum Qualifications: • Must be at least 18 years of age. • Graduate of an accredited school of nursing. • Current Ohio RN license in good standing through the Ohio Board of Nursing. • Must be able to communicate effectively in English, both verbally and in writing. • One year of nursing experience in a behavioral health, psychiatric, or pediatric setting required. Two or more years of experience strongly preferred. Licensure/Certification: • BLS certification (can be provided by Boundless). • Successful completion of a behavioral health de-escalation program (can be provided by Boundless). • Delegated nursing certification, if required by assigned duties (training provided if not already certified). Special Skills and Requirements: • Must be detail-oriented and demonstrate strong written and verbal communication skills. • Must be able to assess and respond to both physical and psychiatric health concerns in a fast-paced residential setting. • Must be able to document in electronic health record systems and use tools and equipment as required. • Must be able to travel throughout the facility and into residential units to provide medication administration and health services. • Must be committed to trauma-informed care, youth and family engagement, and interdisciplinary collaboration.
