Your Responsibilities as a Home Health Registered Nurse (RN):
• Deliver home-based care for patients with a variety of illnesses, injuries, and chronic conditions, particularly after hospital or rehabilitation discharge.
• Collaborate with physicians and lead a multidisciplinary team to provide outstanding patient care.
• Conduct patient assessments, document progress, and communicate changes with the physician.
• Educate patients about their care plan, medications, therapy, diet, and exercise.
• Guide family members on the patient's care plan, medication, and safety at home.
• Ensure that care goals are met and assist with the coordination of discharge from services.
Required Qualifications for Home Health Registered Nurses (RN):
• Graduation from an accredited Registered Nurse (RN) program
• Active, unrestricted Registered Nurse (RN) license in South Carolina or a compact multi-state license
• A minimum of one year of nursing experience, preferably in home health care, Med/Surg, or Critical Care
