Registered Nurse (RN) - Bilingual Preferred (Hybrid, I/DD Waiver Program) in Austin, TX

Job Overview: We are looking for a compassionate and experienced Registered Nurse (RN) to join our team in supporting individuals enrolled in I/DD Waiver programs. This hybrid position requires a minimum of one day per week in the field conducting Annual Nursing Assessments for individuals receiving HHSC funding, with the remainder of the work completed remotely from home. Duties: • Conduct comprehensive annual nursing assessments in compliance with HHSC and waiver program guidelines. • Travel to client homes, group homes, or community settings across the Houston area. • Accurately document findings and submit timely reports. • Collaborate with interdisciplinary teams to ensure quality care and service coordination. • Maintain current knowledge of HHSC standards and I/DD Waiver program requirements. Qualifications: • Active RN license in the state of Texas. • Minimum of 2 years of nursing experience, preferably in community health, home health, or I/DD services. • Bilingual in English and Spanish strongly preferred. • Strong clinical assessment, communication, and documentation skills. • Reliable transportation and willingness to travel locally. Preferred Experience: • Familiarity with HHSC-funded I/DD Waiver programs (e.g., HCS, TxHmL) • Experience working with individuals with intellectual and developmental disabilities.
