Here, you are valued and your work truly matters. A Brief Overview In this vital role, you will be responsible for patient care navigation in a clinic setting. As an RN, you will primarily manage care for patients, focusing on effective education, self-management support, and prompt healthcare delivery. Your duties include developing and monitoring care management processes while supporting primary clinical teams. You will identify high-acuity patient populations and coordinate their care effectively, alongside some patient triage responsibilities. You will collaborate closely with clinic leadership and the medical director to tailor this role to best meet the needs of our patients and teams. What You Will Do • Act as a resource for all clinical teams on care management, educating patients on self-management to optimize their health. • Collaborate with physicians and healthcare providers to identify suitable patients for care management, leading patient care teams effectively. • Work with social services and nursing to establish and communicate appropriate follow-up care plans for patients, ensuring contact to evaluate compliance with health recommendations. • Review and analyze monthly status reports and clinical care delivery information to communicate findings to physician groups. • Provide education to clinic care teams on changing payer requirements and collaborate with payer Case Managers as needed. • Participate in regular team meetings and peer review activities to enhance patient care. • Anticipate the needs of patients, ensuring necessary documentation and pre-visit planning are completed. • Encourage patient self-management and support families in achieving wellness and independence, addressing barriers as they arise. • Complete necessary referrals and oversee the development of educational resources for patient self-management. Required Education, License/Certification, or Work Experience: • Registered Nurse (RN) - An active license in the state of practice upon hire. Preferred Education, License/Certification, or Work Experience: • 1-3 years of clinical nursing experience. • 4-6 years of case management or homecare experience.
