Lee Health is looking to employ an experienced RN to serve as a RN Case Manager within Healthy Start. This RN needs to have 2 years of maternal health care experience as the role involves home visits to follow up with expectant mothers and/or postpartum mother's and babies. Responsible for the delivery of care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Assists with directing and coordinating care for clients based on established nursing practice. The RN case manager will provide services to the Healthy Start client population as outlined in the Healthy Start guidelines. The Healthy Start RN case manager facilitates services across a continuum through collaboration with community resources, the Healthy Start team, and other professional disciplines to ensure effective and efficient delivery of Healthy Start services and the achievement of desired client outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the delivery of services rendered. Contributes to the success of the Healthy Start team through excellence in customer service and performance improvement techniques that demonstrate positive outcomes of said services. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start team and are expected in this role. Duties will include participation in events sponsored by Healthy Start, as well as covering team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our client caseload and the needs of the organization. Requirements Education:Graduate of an accredited school of professional nursing. Experience:Minimum two years of maternal health care experience required. Certification:BLS (American Heart Association / Basic Life Support). License:Currently licensed RN in the State of Florida. Other:Strong interpersonal/communication skills and organized work habits required. Perspective employees must possess the ability to effectively negotiate computerized documentation.
