Registered Nurse (RN), Case Management, Full Time in Chipley, FL

Registered Nurse (RN) - Case Management Full Time Day Shift, Monday - Friday, 8:00am - 4:30pm The Registered Nurse (RN) in Case Management coordinates and oversees patient care throughout the hospital stay to ensure safe, timely, and effective care delivery. This role collaborates with physicians, nursing staff, interdisciplinary teams, patients, and families to develop and implement individualized care plans. The RN Case Manager facilitates discharge planning, monitors resource utilization, ensures compliance with regulatory requirements, and advocates for patient needs to promote optimal outcomes and continuity of care. Education: RN Licensure required Experience: At least one year hospital clinical experience required. Experience in Social Services/Discharge Planning/UR preferred. Licenses, Certifications and/or Registrations: State of Florida Board of Nursing RN Licensure; American Heart Association BLS Healthcare provider certification.
