Notes:
• Work Schedule: Mon – Fri, 8:00 AM – 4:30 PM. No weekends, no call.
• Orientation: 40 hours.

Description:
• Seeking an experienced Registered Nurse Case Manager to support the Adult Stem Cell Transplant (BMT) Program in an outpatient oncology clinic setting.
• This role focuses on care coordination and patient management for individuals undergoing stem cell transplant and oncology treatment.
• The Case Manager will work closely with the authorization department, manage new patient referrals, coordinate appointments across multiple departments, and respond to patient communications via Epic/MyChart and Epic work queues.
• Patient populations include individuals diagnosed with leukemia, lymphoma, multiple myeloma, and those undergoing stem cell transplant therapies.
• The role requires strong clinical judgment, coordination skills, and experience working in a fast-paced outpatient oncology environment.
Qualifications:
• Active California RN license required.
• Minimum 1 year of oncology nursing experience (BMT experience strongly preferred).
• Experience in outpatient oncology or bone marrow transplant settings.
• Case management and care coordination experience in a clinic environment.
• Proficiency with Epic (preferred).
• Strong skills in patient education, symptom management, and interdisciplinary collaboration.
• Experience coordinating authorizations, referrals, and appointments.
• Ability to manage patient communication via Epic pools and MyChart.
• Knowledge of oncologic emergencies, chemotherapy, biotherapy, and symptom management.
• Experience with neutropenic precautions, isolation/PPE, and care coordination.
