We are seeking dedicated Registered Nurse Case Managers to provide exceptional in-home care for former Nuclear Weapons Workers who are facing chronic and terminal illnesses. This role offers you the flexibility to create your own schedule and supports a healthy work/life balance.

Your Role:

As an RN-Case Manager, you will conduct weekly in-home nursing visits for clients, usually lasting 1-2 hours. During these visits, you will perform physical assessments, ensure the safety of the home environment, manage medication planners, and supervise both skilled and non-skilled caregivers. This position allows you to build meaningful, one-on-one relationships with patients while enhancing their health and quality of life.

Qualifications:
• Graduate from a state-approved nursing program
• BSN preferred
• Current, unrestricted RN license
• At least 2 years of nursing experience, including 1 year in home care or a related field
• Preferred: 1 year of supervisory or case management experience
• Current CPR certification

Your Key Responsibilities:
• Provide direct care and comprehensive case management for assigned clients
• Participate in care planning and evaluations
• Collaborate with the regional director and other staff to enhance service delivery
• Act as an on-site clinical resource to meet client care needs
• Ensure compliance with agency policies and scope of practice guidelines
