Registered Nurse (RN) Case Manager Home Health in Comstock, MI

The area of coverage for this position is Kalamazoo County, Cass County, and surrounding area. What You'll Do: • Work with the physician for medication needs and changes to the plan of care. • Manage other disciplines such LPNs, aides, therapy staff to give the best care needed for each patient. • Ability to complete OASIS, recert, d/c, ROCs documentations within an EMR system • Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed. • When appropriate teach and counsel family members about care and processes for disease/injuries. • Attend orientation, meetings and CEUs What We Offer: • Clinician-owned and operated — your expertise shapes patient care. • 4.5-star quality rating with a reputation for excellence and patient satisfaction. • Supportive, collaborative, and patient-focused work environment. • Competitive pay and comprehensive benefits package.
