We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us. We pride ourselves on very low employee turnover. We are looking for a qualified RN Case Manager who wants a long‑term position with a great company. Responsibilities As a Hospice and Home Health RN Case Manager your duties will include but not be limited to: • Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care. • Create patient centered goals with patients, family members and providers • Assess, supervise and direct patient care • Develop plans of treatment • Ensure chart compliance with state regulations • Coordinate patient care with physicians and other providers Qualifications Hospice and Home Health are both challenging and rewarding!
