Job Description & Requirements • Specialty: Nurse Navigator • Discipline: RN • Duration: Ongoing • 40 hours per week • Shift: 8 hours, days • Employment Type: Staff Location: Main Office Location: Medical Plaza One, 9800 S Healthpark Drive,Fort Myers FL 33908 Department: Lee Physician Group (LPG), Community Care Outreach Work Type: Full Time (80 hours bi-weekly) Exempt/Salary Shift: Days, 8:00 AM to 4:00 PM, Monday - Friday Work Week, No Weekends, No Holidays Minimum to Midpoint Pay Rate: $35.04 - $47.30 / hour BSN Degree and a minimum of three (3) years of Clinical Nursing Required. ACM (Accredited Case Manager) or CCM (Certified Case Manager) or NACCM (National Academy of Certified Care Managers) or CMCN preferred. This is a hands-off RN role. Work from Home/Hybrid position. EPIC/Strong Computer/Telephonic Skills Needed. Patient Education. The Nurse Navigator RN will visit patients in various locations (examples: in home visits, hospital, doctor's office, skilled nursing facility, library, cafe...etc.) Must be living in an easy commuting distance from Lee Health/Lee County areas. Summary Responsible for providing team leadership for the assessment, triage, care coordination and coordination of the disease education to improve health outcomes for specific moderate to high risk based patient population and wellness activities for low risk patients. Works collaboratively with high risk, underserved patient populations. Connects with the patients in person (through home visits), on the phone, on-site in the physician offices, hospital, SNF, and other sites of care. Provides face to-face interaction with patients and their care team when appropriate to improve patient care in person or via telehealth or video visits as needed.
