Responsibilities
Your Role:
• Oversee patient care plans, ensuring adherence to medical regulations and home health standards.
• Coordinate patient admissions, discharges, and transfers while maintaining accurate documentation and medical records.
• Collaborate effectively with field nurses, aides, and support staff to assign cases, manage schedules, and oversee timely completion of patient visits.
• Provide essential education to patients and families on home health services, treatment plans, and medication management.
• Assist in developing and implementing quality control measures to ensure outstanding patient care and compliance.
• Work closely with the billing department to review patient records, confirm proper coding, and address any billing or insurance issues.
• Maintain accurate patient files, clinical documentation, and compliance reports as required by state and federal regulations.
• Help create and update company policies and procedures reflecting best practices in home health care.
• Act as the primary contact for patient and family inquiries, addressing concerns and resolving issues promptly.
• Assist in training new clinical staff and provide continuous education on documentation standards and patient care protocols.

Qualifications
What You'll Bring:
• Professional, ethical, and respectful work approach.
• Capability to provide personal care for clients, manage Hoyer lifts, maneuver wheelchairs, and assist clients with community activities.
• Excellent communication skills, both oral and written; strong interpersonal abilities.
• Proficient problem-solving capabilities, demonstrating good judgment.
Education and Experience:
• Current RN license.
• Valid CPR/First Aid certification.
• Minimum of 2-3 years nursing experience, preferably in home health or clinical administration.
• Certification in Advanced Cardiac Life Support (ACLS) and/or Pediatric Life Support (PALS) may be required.
Management Authority:
• Ability to train other associates.
