Job Title: Registered Nurse (RN) – LTC / Rehab Department: Nursing Reports To: Director of Nursing / Nurse Manager Job Summary The Registered Nurse (RN) provides professional nursing care to residents in a long-term care and rehabilitation setting. The RN is responsible for assessment, care planning, medication administration, supervision of nursing staff, and coordination of resident care to promote optimal health outcomes. Essential Duties & Responsibilities • Assess resident condition and develop, implement, and evaluate care plans • Administer medications and treatments per physician orders • Monitor resident status and respond to changes in condition • Supervise and support LPNs and CNAs • Coordinate care with physicians, therapists, and interdisciplinary team members • Perform wound care, IV therapy, and other skilled nursing procedures • Ensure accurate and timely documentation in the medical record • Participate in admissions, discharges, and care conferences • Educate residents and families on care plans and treatments • Ensure compliance with facility policies, infection control, and regulatory requirements • Respond to emergencies and provide appropriate interventions Qualifications • Current RN license in good standing • Graduate of an accredited nursing program • LTC or rehab experience preferred (new grads may be considered) • Strong clinical, leadership, and communication skills • Ability to multitask and work effectively in a fast-paced environment
