Registered Nurse (RN) – GAPP Intake & Documentation (Non-Skilled Home Care) Company: Harmony Care Solutions Location: Atlanta Georgia (Remote/Hybrid with local assessments as needed) Job Type: Part-Time Pay Range $75 Job Overview Harmony Care Solutions is seeking a Registered Nurse (RN) to support our non-skilled pediatric home care services under the Georgia Pediatric Program (GAPP). This role focuses on intake assessments, documentation collection, and compliance approval, not hands-on skilled nursing care. The RN will act as the clinical authority ensuring all clients meet GAPP eligibility requirements and that applications are complete, accurate, and approved. Key Responsibilities • Conduct initial RN intake assessments for pediatric clients applying for GAPP services • Collect, review, and approve all documentation required for GAPP eligibility and approval, including: • Physician orders / Plan of Care (485) • Medical necessity documentation • RN assessments and supporting clinical notes • Service justification for non-skilled care (personal care, companion, sitter services) • Ensure all documentation meets GAPP, Medicaid, and state compliance standards • Coordinate with physicians, families, case managers, and internal staff to obtain missing or corrected documentation • Submit completed and compliant applications for GAPP approval • Participate in reassessments and annual recertifications as required • Maintain accurate, organized client records • Provide clinical oversight for intake and documentation only (no routine skilled bedside care) Qualifications • Active Registered Nurse (RN) license in the State of Georgia (required) • CPR certification (required) • Experience with pediatric care, home health, or Medicaid/GAPP programs preferred • Strong knowledge of documentation, compliance, and care coordination • Excellent organizational and communication skills • Ability to work independently and meet application deadlines Important Notes • This role supports a non-skilled home care agency • No ongoing skilled nursing shifts are required • Focus is on intake, compliance, and documentation approval What We Offer • Flexible schedule • Competitive pay per hour or per assessment • Administrative support team • Opportunity to make a direct impact by helping families qualify for essential care Job Type: Part-time Pay: $75.00 per hour Expected hours: 10 – 25 per week Benefits: • Referral program Work Location: Hybrid remote in Douglasville, GA 30134
