As a Registered Nurse RN in Home Care, you will play a crucial role in coordinating patient care. You will serve as the primary contact for all disciplines involved, ensuring seamless communication and care delivery. Key responsibilities include:
• Performing patient visits and overseeing the frequency of care
• Consulting with physicians regarding patient care
• Accurate OASIS collection and maintaining current medication profiles
• Reporting lab values to physicians in a timely manner
• Attending weekly case conferences and monthly case manager meetings

Qualifications:
• Currently licensed as a Registered Nurse (RN) in your state of employment
• Preferred minimum of one year clinical experience
• Demonstrated knowledge and skills in current nursing practice
• Valid driver's license and automobile liability insurance
